The information provided by the applicant will be used to apply for the enrolment of the “Digital Learning”. According to the “The Commonwealth’s Privacy Act 1988 “, you have the right to request access to and correction of the personal information provided. If you have any questions about filling in your personal information, including requesting to check and change the information, please contact our staff. We promise to comply with the provisions of the “The Commonwealth’s Privacy Act 1988”, and to exceed the internationally recognized personal data protection level where possible. In order to fulfill this commitment, we will ensure that our staff comply with the strictest regulations on security and confidentiality.

I request that the attendee named above take part in the “Digital Learning” held at Hillview Intercultural Community Centre, 1-3 Hill View Pl, Bentley WA 6102 between 7 November to 5 December 2024, 10am-1pm, and agree that staff and leader of the program are free from all responsibility for accidents and loss of property in connection with the attendee’s participation.
